Try using a Koss brand inner ear type. They are a little pricey but the sound quality is superb and they fit snug. They fit because they come with several sizes of ear cushions. Ear buds are pretty ridiculous. They're not really made to fit ears, I think. Especially the iPod ones. Unfortunately, if you want ones that stay in your ears, you're going to have to try a variety of them and find the ones that work best for the unique shape of your ears.

There are some ear buds that have little strips of plastic that go around behind your ears, so they stay in your ears and they can't really fall out. I'm not sure what they're called, but I know Best Buy and WalMart have them. I don't think they're ridiculously expensive, either.

Well, ear buds are generally manufactured in a standard size. You may need to purhcase ear buds that fit your ears accordingly. I highly reccomend headphones because they will stay comfortably for a long period of time.
