Respectfully remind the principal of your commitment to graduate with your class even with your struggles with your health. It is an important rite of passage to adulthood and it would mean a lifetime of memories to you and your family if you would be allowed to participate.
Tell him your plan to minimize health issues before the ceremony and your back-up plan in the event you do have an event. (Talk to your doctor first and have any medications or devices with you at the ceremony. Perhaps she/he will even write you a note of support to include with your letter.)
Perhaps his reticience to permit you to be there is the concern your health issues will interfere with the event for the rest of the graduates. If you can show him that you will be prepared and ready to step out if necessary, perhaps he will change his mind.
