Don't call it anything. Put it between parentheses (adopted English name) or quotes (nickname) :

I would use "familiar name;" "nickname" sounds too informal for many work situations. I just use my formal name on business documents, and then tell coworkers and colleagues I prefer the more familiar version of my name.
