he scratches himself to where there no hair in spots and scabs? what causes this and what to do?

Since he is indoors only, you could probably rule out any type of skin mites.
It could be food allergies.
Either way, he needs to be seen by a veterinarian for a proper diagnosis and treatment plan.

Very likely to be fleas - do you flea treat him?

It's myth that indoor cats do not get fleas. Fleas can enter houses too!

You need to get some proper flea medication from the vet, your poor cat sound very uncomfortable, so do it as soon as possible. If you've been using flea stuff from shops, you can now see that they don't work. You need something like Frontline

Definately sounds like some kind of an allergy, possibly to food. There is food for cats with specific allergies that is made by Science Diet. You should definatley see your vet so that they can do some tests to find out what is going on with him.
