Environmental Medicine and/or Functional Medicine would be the BEST groundwork for a gastroenterologist. Most gut disorders are of a functional nature and caused by environmental exposures.

Gastroenterologists must finish a residency in internal medicine before they start their training in a gastroenterology fellowship, but there's no great need to go into private practice for any period of time in between, and few do. That's fine for most, although sometimes one wishes subspecialists had some experience in primary care, when they have the personality that won't allow them to see the point of view of the generalists who refer their patients to them.

You will need to complete an internal medicine residency before training in gastroenterology.

There is no need to continue with IM after your GI fellowship is done. You should be busy enough with GI cases. Practice what you trained for.

There is, of course, some overlap. You may find that you end up treating non-GI problems in some patients just because they like and trust you.

It wouldn't be a bad idea. This way you get a feel if it's something you really want! I say go for it.
