Any time the registry is altered or changed, there's a possibility something bad can happen. I've been using this free product for a couple of years now and I've never had any bad experiences:

The registry cleaner automatically prompts you to make a backup. I always do, but I've never had to use it. My only suggestion with this (or any other product) is to read all the help info, study the tutorials, and use the recommended default settings the first few times until you become familiar with the product.
