what should i do when i download spybot should i press run or save?

Either. When you go for, run, it'll be able to fetch the latest updates right away. (Remember, you need to go fetch updates at least once a week, to keep them current.)
If you were to click Save then you might have to go find the downloaded installation file and "run" it to have it actually do the installation. That's all the "run" is - run the install now, or "save" it so's you can "run" it later.

If you need the installation file for future use click save. If you are sure your do not need to use the installation file any more in the future just click run.

If you're trying to get the program, download it -- this means Save to your computer first. Then run it from your computer.
