I have already looked into the ada.org but i can't seem to find where to make a formal complaint.

The ADA has nothing to do with accepting complaints, but the state components do have dispute resolution boards.

If you want to file a formal complaint, you have to do this with the "State Board of Dentistry" in the state where the offense was committed.

I just got a violation for posting an innocuous answer to someone's really stupid question (I believe all I said was, "Wow. No comment."). Seriously? What probably happened is that two idiots took the stupid question seriously and didn't like the fact that I thought it was stupid. So they reported my answer in some attempt to prove how smart they think they are. Honestly, I don't think there's any person actually making these decisions, it's just some computer that figures it all out. So, the fact that you got denied on your appeal doesn't surprise me at all. I kind of think the appeal is for show, to make you think that there is some sense of rationality at work here. My point is that there is not. Am I going to appeal my violation? No, because it's not worth my time. In the end, it's just a website. It's not like anyone's livelihood depends on it. If they violate you a whole bunch of times, then you just close your account and open a new one under a different user name. The point is to have a bit of fun, occasionally blow off some steam, etc. When it stops being enjoyable, then it's just time to stop. Am I annoyed that I got a violation for some petty BS? Yeah. Do I really care all that much in the grand scheme of things? Not really. At some point, enough decent people will get violated for stupid reasons and will stop using the site, and then it will be left to the people with absolutely nothing to say. Oh well.

Just write a letter and send it to the ADA. Make sure to send it registered. If it is a serious issue, you might want to get an attorney to sue him for malpractice, etc...
