My 2 yr old daughter is spending the weekend with her and we are going out of town. does anyone have a type of form i need to write out for my mother incase my daughter gets injured and needs surgery, or medical attention? i am leaving her insurence cards and know i need to do a waiver as well. thanks!!!

Write out a letter that goes something like this:

I, the mother of authorize to seek medical treatment and make decisions regarding the care of my child for the period of .

Sign and date it - to be double safe, have it notarized if that's possible. You might also want to include a copy of your photo id and your mom's id.

I have temporary custody of two of my brothers kids and when I got them their mother just wrote out a letter on a piece of paper stating that I had permission to seek medical attention if needed.
