Well, what you say depends on what sex you are. If you are a guy, you need to let her know that she doesn't look as bad as she thinks she does (even if she really does look as bad as she thinks she does).

A woman can respond saying things like "yah, I hear ya", "been there" or something along that line.

It is true that women think they look fatter than they really are and need to be told they don't look that bad by men.

It depends on how well you know the woman.

If she is friend, family, or co-worker, you smile and say, "You look great just the way you are!" (It doesn't matter if you are lying, just say it!)

If it's someone you don't know very well, smile and change the subject. Quickly!

It's either vanity, and she's fishing for compliments, or it's insecurity, and in that case it doesnt matter what you say, because it wont change their own opinion anyway...

Just stay quiet shrug, and brag about how good your metabolism is lol :)

There is no right answer, think of it as a rhetorical statement. And if you make eye contact right after they have said it, take two steps back and mumble something and smile.

then compliment her.
