Simply put, it's what you believe in a nutshell -- not a whole lot of lengthy explanation about "why". It's a statement, not an essay. The Apostle's Creed and the Nicene Creed -- easily found on a quick web search -- are really good examples.

Where to make a statement about faith you should live your life so people can see "your good works and glorify your Father in heaven"
Now if your actually writing a paper on it. Start with what faith means to you. then talk about times where you had to exercise faith or someone else's experience, then state your own testamony

It can be as long or as short as you like.
If you are CLEAR in knowing the purpose of one, it should be easy.
The shortest one I ever read was simply this: I believe the Bible.
The longest I read just dragged on so much I don't know why they didn't just copy the Bible completely.
A statement like this usually covers certain things like what you believe about: man, Jesus, the church, Holy Spirit, Mission of the Church, whatever you want.

1) What you believe
2) Why you believe it (scripture proof)
3) When you came to believe it
4) Your goals
