All forms of transportation account for 14% of pollution, road transportation in itself is 9%. I don't have specific figures for cars but if you were to say half of all road traffic pollution came from cars then you're looking at a figure of 4.5%.

The number of deaths caused by air pollution is very difficult to estimate. Air pollution is often a contributory factor in respiratory conditions but there are other factors as well. Because everybody is breathing the same air there's no control group with which to make comparisions. The Breath-taking report in 1996 estimated there were 64,000 deaths per year attributable to air pollution. http://www.nrdc.org/air/pollution/bt/btinx.asp

As for the price of gas - Gasbuddy puts the average cost today at $3.01 a gallon, varying from $2.82 in NJ to $3.41 in CA. http://www.gasbuddy.com/
