Lots of medical conditions can cause blood to appear in a person's stool. They range from the very minor to the very serious, and can include:
Anal fissures Small cracks or tears around the anus that can be caused by large and/or forceful bowel movements or anal sex (including insertion of fingers, sex toys, etc.)
Hemorrhoids (a.k.a. piles)
Enlarged veins in the lower part of the rectum or anus
Infection or disease in the GI (gastrointestinal) tract
Bacterial infections, stomach ulcers, or inflammatory bowel disease (IBD)
Tumors or cancer of the GI tract
Polyps or colon cancer
Only a health care provider can determine whether the blood in your stool is from straining too hard, or from a more serious and/or long-term medical problem. People who experience blood in their stool or on the toilet paper after a bowel movement occasionally need to seek medical advice to see if there is a problem. Because small amounts of blood may not necessarily mean a small problem — for example, one of the early signs of colon cancer is minute amounts of hidden (or occult) blood in stools — any amount of blood in the stool needs to be followed up with a call to your provider. S/he can then decide how deeply the problem (and your GI tract) need to be examined. A large quantity of blood (say enough to color the water in the toilet pinkish-red or show up as visible clots) can be a sign of a life threatening bleed in the GI tract, which requires emergency medical treatment.

The answers above are all good. Here's a "different" possibility:
A friend of mine once drank a soft drink out of a glass bottle. Several hours later when having a BM he noticed blood.
This kept happening for a few days so he went to the doctor.
They found out he had gotten some chips of glass in his system, probably swallowed from the soft drink bottle!
You may have gotten a bit of something with your food or drink that is making your rectum or lower bowel bleed.
If it doesn't clear up in a few days you MUST get medical attention. Bright red blood is actually better, though. It usually isn't cancer, just a hemmorhoid or colon polyp.

bright red blood usually indicates internal hemmorhoids, taking a stool softener and drinking lots of water will generally clear this up.
If the stool is black, or the blood is in clots and appears old, then the bleeding is higher up in the intestinal tract and could indicate serious problems which must be looked into by a physician.

If the blood is fresh, red and without clotting. It may be Haemarroids which is the name given to veins that hang outside the anus through straining or physical exertion.
If the blood is black, and offensive smelling with clots. It may be a digestive tract disorder that needs looking into immediately.
Either way, you should see your doctor. Bleeding from the anus is not a good sign.

Depends.... is it bright bright red?
It could be from hemorrhoids. You should take sits baths in hot water to reduce swelling. Go to the doctor for medication to reduce the swelling.
Is it deep dark red and smells really really bad?
It could be a GI bleed. In that case you need to go to the doctor ASAP

See your doctor. It's possibly colorectal polyps, which can become cancerous. it is treatable, but time matters, so don't mess around.
If you see black, tarry looking stuff in your stool, it's most likely ulcers.

could be from wiping to hard, or a hemroid that exploded. Either way I would suggest that you see a doctor asap...you don't want to mess with that, it can be serious.
