The Bible is God's written Word to us.
It was written by human authors, under the supernatural guidance of the Holy Spirit.

“Above all you must understand that no prophecy of Scripture came about by the prophet's own interpretation. For prophecy never had its origin in the will of man, but men spoke from God as they were carried along by the Holy Spirit.” 2 Peter 1:20-21. (New International Version)

Depends on who you're asking. Christians(like me) would say that God did. He inspired people like the apostles and prophets, and they wrote the words down.
In other words, the actual letters were written down by Moses, David, Solomon, Isaiah, Jeremiah, Danil, Matthew, Mark, Luke, John, Paul, etc.
The content is from God.

The 39 little books that go to make-up the OT, were assembled and maintained by Jewish priests and scholars.
That's what Romans 3:1,2 says:
"First of all, because they were entrusted with the sacred pronouncements of God.. . ."
The NT is mostly letters to christian congregations. They were kept and the gospels and Revelation were added to it.
Most likely this was carried out by the Apostles and 'older men' of Jerusalem. Acts 15:6

Well, a bunch of different people wrote a bunch of different sections. God told each one of these people what to write and they did. You have to remember that the Bible covers a huge time period so one person couldnt have been there for it all

The author of the Bible is God: "All scripture is given by inspiration of God, and is profitable for doctrine, for reproof, for correction, for instruction in righteousness:That the man of God may be perfect, throughly furnished unto all good works. II Timothy 3:16
The writers were many including the apostles, some kings as well as prophets inspired by God. You will see some of their names in the Bible. Some include Jeremiah, Moses, David, Solomon, Daniel, Peter, Paul, John, etc.

As far as translations:
Martin Luther of Germany was responsible for making scripture available to the people in their language as the Catholic Church wanted kept the people in darkness (withouth the Bible to read for themselves).
The one responsible for the English translation that is trust-worthy was King James, here is his biography:

Eusabius in th 4th Century was first to complie books of the Bible. Then the council of Nicea in the middle of the 4th century was when they compiled the writings into the common Bible.
