Ultimately, it's whatever the law is in the state you're driving. Ask a cop, they'll tell you.
I know in MA and RI, you don't have to use a turn signal in a turn only lane. It's a given you're turning. It was upheld in court, though I can't cite the case because I don't have the time to research it.
Any decent lawyer can get a turn only citation overturned if you've been ticketed.

Normally best to leave the turn signal on until you complete your turn. The exception being a long semi-divided right or left turn lane that has its own traffic light, then it would be ok to signal your lane change into this particular lane and wait for the green arrow.

Two things. First, did you signal to get into the turn only lane? Then leave your dam blinker on until you turn. Second, I have seen more pinheaded idiot brain donors drive straight through those lanes instead of turning than I care to remember. Not only that, the same pinhead idiot brain donors drive for miles with that dam blinker on, even going so far as to change lanes the opposite direction with it on. So how am I supposed to know what they intend to do? Guess? Next you'll want to make brake lights an option because "it's obvious my car is slowing down".

Yes. As Kaivon said, oncoming traffic need to know what you plan on doing. If you're sitting in a turn lane, other vehicles may not know you're in an exclusive turn lane and need to see your turn signal to know what you plan on doing.

I have wondered about that too. I am in the habit of using
my turn signals, so I do it automatically. I have watched the cars in front of me and often they aren't using theirs. I will do
it because it wouldn't be worth getting a ticket.

Yes, because the turn signal isn't only for the people behind you... it's also for oncomming traffic. It's a DOT law.
