$76 for unlimited subway and bus trips for 30 days. You can purchase the card with a credit card from a machine that is located in every subway station. It's a good idea to buy the MetroCard with a credit card - that way, if you lose it, the MTA will issue a new card with the amount of time that was left on it. Read this:

a monthly unlimited subway pass is $76 and yes you can use your credit card. You have to go to the metrocard vending machines in any subway station.

I think you can pay with credit cards and you can find them in any subway.(I don't live in the city, I live on the Island but I've been there quite a few times.)
