I've lived in Holland where you can find a good/safe cycling path on the side of most major roads and there is no need to own a car.
I am easily in shape to go 12 miles to work and 12 miles back without breaking a sweat...or up to 70 in a day so shape is not the limiting factor keeping me in a car in Houston (where the path setup along major roads, aside from Braeswood Bayou, is a death wish for cyclists).

Are there any cities in the US which in any way approach Holland's level of seriousness/convenience for cycling?

Most bike publications will cite Portland, OR as the most bike friendly urban area in the country. The bike is just a way of life in Portland. Bikes are everywhere. Bike racks are everywhere and all roads have bike lanes. Public transit is also equipped to take bikes on board to make travel door to door and not require cars.

Austin Texas is Very bike freindly, also many Bike lanes for urban commuting ....

Santa Barbara CA is great! lots of bike lanes and complete roadways (with lanes for buses, cars, AND bikes!)

san francisco, ca and davis, ca have many roads with bike lanes and a large cycling population.
also, denver, co is big. .

holland, michigan USA is great. its a town on lake michigan thats great for cyclicts. although its not very big, they do have a nice system. so does portage michigan.
