What exactly is Substancial pain?

Well I have never heard of "substancial" pain. But I think you just spelled it wrong and you meant substantial pain.

Substantial just means in excess or a lot of...

So substantial pain would be a lot of pain or something that is very painful.

substancial pain is considered above level 3 on the pain scale. The pain scale goes from 1 to 10 ten being in tears and screaming very loudly, 1 being little or no pain. There are faces associated with the pain scale. Usually the hospital has those posted.
