Well, O'Brien is the second most common name in Ireland, and it's an outside chance that they are all related to each other.

The name comes from Brian Boru, who was the last High King of Ireland, who died in 1014 getting rid of the warring Vikings. It wasn't that he had a lot of descendants (who may have started to call themselves O'Brien), but as High King he had a large retinue who called themselves O'Brien as they were with him, in a manner of speaking, and their descendents kept the name.

I know 4 families with this name who are not related to each other that they know of.

It's in the family. O'Brien means descendant of Brian. Don't know how many Brian's had descendants. Probably multitutdes.
