Aso to the question about it disrespecting the relationship, I don't think so. I think that if you don't want to have a threesome, you should have the courage and confidence to say so, and if he does one one, he should have the courage and confidence to say so. It's been said a thousand times, relationships are built on communication. If he's hiding a secret fantasy, that's not communication. If you know what he's thinking, even if you don't want to do it, then you can at least talk about why and what you do want to do.
For example, my wife wants me to go shoping with her and her friends. But I don't enjoy shoping. Is it disrespecting our relationship for her to ask me to go shoping with her and her friends? Or how about this: is it disrespecing our relationship for her to find Sean Conery attractive and tell me so? Or how about this: is it disrespecting our relationship for her to ask me to do the kinds of things she fantasizes about, like make love in the rain, even though I have no interest in them? Amazing how often it goes from "disrespecting the relationship" to "perfectly natural" when the shoe is on the other foot.

Not at all, he is just trying to keep things heated. This is quite common, in fact, if you check with your girlfriends, you might just find out that they have similar fantasies either with 2 guys or 2 girls. It is not disrespecting a relationship as it is just a fantasy. It is no different to you looking at any male actor and going phwoar!!! As long as it is never sought after to happen for real then there is no harm.

having a fantasy is not acting on the fantasy --- he is not disrespecting his lady nor the relationship --- if he acts out the fantasy then yes he is disrespecting and endangering the relationship
