Using APA Style, put the numbers bottom center and do not put the title on every page.

If this is for a college thesis, check with your instructor about the style they want you to use.

For example, half way through my wife's dissertation process, the rule changed from "page numbers at right top corner , except for the first page of a chapter which was bottom center numbered" to "bottom center for all page numbers."

See what I mean? Better check with your professors.

MLA?
If so, your last name and the page number is in top right corner of each page--1/2- inch from top and 1 inch from right.
