Based on the limited number of facts you've given us, it could be just about anything. All we know is the patients age, his sex, the fact that he was under emotional duress (which may or may not have been a factor), and that he coughed up blood. How much blood? Spittles? Pints? Was it from his stomach? His lungs? His throat?
I'm sorry, but without a physical exam, there is no way to diagnose your friend.
And anyone who would post a diagnosis would be acting irresponsibly and shouldn't be trusted. No real doctor would.

that is very dangerous, he should overcome his fear and go! I'm not a doctor but it might be something in the lungs or liver. I'm am so serious he needs to go to the doctor. If you are his true friend and care about his life you should make him go before it gets worse. Coughing up blood is not normal.

He's an effing retard if he doesn't go to the doctor because throwing up blood is serious! Probably ulcers but you never know.

Generally a person can throw up blood once they have thrown up so much there is nothing left in their stomach except for the bile which is acidic and can damage the stomach and lining of the throat when it is coming out of the body.
If your friend is in a relationship that makes him so upset he is getting physically ill, he needs to rethink this relationship. He can cause scars to form in his digestive tract if he keeps reacting in this fashion.

Possibly a bleeding ulcer or esophageal varices. If he threw up more than a spot of blood, then he needs to go see a doctor right away, it's not something to play games with. He also needs to pay attention to his bowel movements. If they're tarry and black, that's an indication of blood in your stool, and also a bad thing.

Tell your friend if he doesn't go to a doctor he could possibly die. He could have a bleeding ulcer, esophagus, or something else. I am not a doctor, but I know that if I puked up blood or knew someone who did, that it requires medical attention IMMEDIATELY.

AS young as he is, it may be an ulcer. In any event throwing up blood is not good and he should see a Dr. or he may afraid of a lot more in the future

Tell him he has to go to the doctor 'cause it can be really dangerous, and if he said no, tell him you're a friend who really cares about him and for his own safety he should go to the doctors.
