If wearing a "modern" kilt (pre-sewn, etc.) and it has built in straps you don't need a belt, but the outfit looks much better with one (and a sporran, of course!)
When putting on a traditional greatkilt, which is a 9 yard strip of tartan wool, you must lay it out, hand pleat it, and strap it on with a belt. A lot of work, but worth the effort.
And remember: If you wear your kilt above the knee, you're a wee little boy.
If you wear your kilt at the knee, you're a man.
If you wear your kilt below the knee... you're a damned liar! :)

Never wear a belt if you're wearing a waistcoat/vest. Lots of kilt shops display both on a model at once, but this is wrong - it makes the waistcoat ride up and hides the belt.

It is always correct to wear a belt with a kilt if the kilt will not stay in place otherwise.

In many instances, the belt is essential to keep the kilt in place. They are made in leather with or without patterning. It is always appropriate to my knowledge, although the type of belt might depend on the formal-ness of the occasion.

A kilt comes with straps and buckles to keep it in place. You don't wear a belt with it - ever!
