It means both the trim level and the type of engine.
Ford originally introduced their 1.6 to 2.0-litre engine range as "Zeta", and planned to use "Sigma" for the smaller engine range, but problems with other manufacturers having already used those names as model names meant they changed the engine names to "Zetec" and "Zetec-SE".
They then started using "Zetec" as the trim-level name for sporty mid-level versions of their cars.
So the name is used for both, and just because a car is "Zetec" trim level does not mean it definitely has a "Zetec" engine (it may have a Duratec, which has now replaced the larger versions of the Zetec engine, or a Duratorq, which they use for most diesel engines).

the SE is now known as "duratec"
cars started being badged with "zetec" trim around 2000. A
notable car is the Fiesta Zetec-s, a sporty varient. buy one.
A zetec badged car can have any engine under the bonnet, these days it is a Duratec, Endura E or Duratorq (tdci)
though the zetec itself is no longer in production.

Zetec is a family of 4-cylinder engines built by Ford. Other engine families from Ford includes Duratec (V-6's, available as 2.5, 3.0, and now 3.5L), Powerstroke (Diesel V-8's), Triton (V-10 gas) and more.
It's possible that some European Ford car models include the engine family name in the full model name, in order to distinguish them from their plebian cousins, but Zetec itself would still refer to the engine.
And it has NOTHING to do with Honda's V-TEC. Honda give letter and number designators to their engines, like all other Japanese car makers. They don't give them "names" like Americans do. V-TEC stands for Variable valve Timing and Electronic lift Control, not to any specific engine or family of engines. Technically speaking, all of Acura's V-6 engines are V-TEC engines as well, and are labeled as such
EDIT: Note to Barbara, wikipedia's article on Ford Focus (International) says that only Mk 1 Focus had Zetec, and they are all petrol engines. Which means Zetec is the engine, not the trim.

A bit of both. Zetec is the name for that family of engines, but some Mondeos came as "Mondeo Zetec" with a few extra toys fitted as standard.

Zetec is the type of engine I have come across 1.25, 1.4, 1.6 1.8, 2.0 and 2.5 v6 zetec engines. In escorts, fiesta, mondeo, puma, focus etc. Not all ford engines are zetec though.

Buy on condition rather than mileage. An average car does about 10,000 miles a year, and if it's well looked after, serviced regularly and not abused, will last for 150k+miles. I actually wouldn't want a 2004 car (getting on for 4 years old, remember) that had only done 7000 miles. If it's been sitting idle for months at a time or worse, driven only 2 or 3 miles a day all its life, then it could have serious problems.
