Try Microsoft InfoPath.

It is quite a nice software for both paper based form (easy to format and simple to design) and for web based forms.

Most forms for printing are just Tables. Web forms are a totally different animal because they can be filled out online.

In Word, click on the Tables Tab and search for: Table. Stuck? Click on the Help Tab and search for: Table

Adobe Acrobat has some advanced programs that many people use. They can be interactive.
