Make sure when you are taking her outside that you use the same door all the time. Praise her like crazy when she does her toileting outside. Try not to tell her off when she goes inside unless you catch her in the act, then pick her up and just say "Outside" in a grumpy voice and take her outside to finish. After a week or so of being consistent (taking her out every 2hours) she should start to to go to the door, you will have to watch as she may not whimper to go out. Praise her when you see her go to the door and let her out, lots more praise when she goes to the toilet. She will get the hang of it soon. Just be patient and consistent.

I know you are very frustrated and feeling like a failure, but at this point I would get a crate and start crate training again. If you do it for a few months she will be ok. It will be hard at first, because they will try anything to make you feel guilty about putting them in a crate. I am going through that now with a new shih tzu puppy. Good luck you just have to say: I am smarter, and bigger than this dog and she will bend to my will eventually.

Sandy has a point crate her when you can't watch her and take her out immediately when letting her outta the crate. go with her and PRAISE when she pees outside (or on puppy pad if that's the goal - stick to one too).
To have a clear "LET ME OUT" message hand a bell off the door handle, jingle it before you open the door (EVERY TIME) coax her to tough & jingle bell and then imedatally open door. she'll get the 2+2 (bell jingle opens door)
