We use a chart with faces on it. They go from smiling, happy and laughing all the way along to crying and miserable.
The kids just point then to how they are feeling.

There are so many charts for kids and even adults.

You could make one yourself easily enough.

If the child is so young they are unable to do this, you have to use your own judgement and reconise when the child is not acting 'normal' and showing signs of distress.

Use the smiley face chart, also watch their behavior. Generally you can tell if someone is in pain...are they less active? Do they cry a lot? Does their face just look like they are in pain? Do they protect certain areas of their bodies? Many different signs to look for, just watch their behavior.

A lot of hospitals (not sure about dr's offices) are starting to use a chart that has pictures on it that people can point to in order to describe where it hurts.

Check with the American Academy of Pediatrics as well to see if they have additional info on this.
