Yes you should comply, those are his wishes. He can have his tombstone made ahead of time if he so wishes.

If it bothers you, then talk to him and explain that you or other friends and family will be the ones who will go there and see the tombstone.
If he still wants it, then perhaps it is his choice, but in the end like you said he will not be there to decide.

Have you ever seen Boot Hill Cemetery in Arizona, they have some of the funniest inscriptions I have ever seen on tombstones.
Even if he isn't here to see it, wouldn't you think it disrespectful not to fulfill the wishes of the person you love for the sake of what the 'neighbors' will think?
