He then tried rubbing it down with heat ointment. Then all of a sudden the pain shot up his leg. So much pain that he became immobile for the rest of the evening. He took some Tylenols and in the morning he was alright. What could it have been. He described it like a ball building up on the outside of the foot in front of the ankle socket. Can this be an indicator or something.

It's possible that this is bunion starting to form. I recommend your friend see a foot specialist, Podiatrist, as soon as possible and get a thorough examination. It's best to stop something before it a becomes a major problem.
