Blood in the stool can be bright red, maroon in color, black and tarry, or occult (not visible to the naked eye). Causes of blood in stool range from harmless, annoying conditions of the gastrointestinal tract such as hemorrhoids to serious conditions such as cancer. Rectal bleeding (known medically as hematochezia) refers to passage of bright red blood from the anus, often mixed with stool and/or blood clots. Therefore, melena usually signifies bleeding is from the upper gastrointestinal tract (for example: bleeding from ulcers in the stomach or the duodenum or from the small intestine) because the blood usually is in the gastrointestinal tract for a longer period of time before it exits the body. Sometimes melena may occur with bleeding from the right colon. On the other hand, blood from the sigmoid colon and the rectum usually does not stay in the colon long enough for the bacteria to turn it black. Rarely, massive bleeding from the right colon, from the small intestine, or from ulcers of the stomach or duodenal can cause rapid transit of the blood through the gastrointestinal tract and result in bright red rectal bleeding. In these situations, the blood is moving through the colon so rapidly that there is not enough time for the bacteria to turn the blood black.
Sometimes, bleeding from the gastrointestinal tract can be too slow to cause either rectal bleeding or melena. In these patients bleeding is occult (not visible to the naked eyes). The blood is found only by testing the stool for blood (fecal occult blood testing) in the laboratory. Occult bleeding has many of the same causes as rectal bleeding and may result in the same symptoms as rectal bleeding. It is often associated with anemia that is due to loss of iron along with the blood (iron deficiency anemia).

Blood from the back passage can be a serious symptom, but when it is slight and intermittent and is only present on wiping, then it is probably due to a small pile. This can otherwise be quite symptomless. Bleeding that would be a cause for concern, would be mixed with the motions, and would probably be accompanied by some mucus. There may also be some disturbance of the bowel actions, which would no longer be regular. So I don't think you have anything to worry about. However, I take a very cautious view and believe that it is better to be safe than sorry and most doctors would agree with this. Anyone with rectal bleeding should report it to their GP, who can check the facts and then usually do a simple examination to make sure there is nothing more to it. Then you can go on your way completely sure that there is no problem. So I advise you to remove all doubt by telling your GP about this and I am sure you will receive reassurance, and feel happier for it.

I wouldn't worry about it. The blood is just an indication of some tearing of the rectal tissue. So long as it's only on the toilet paper, and not on his briefs, I wouldn't consider it serious. Perhaps you could council him not to wipe so hard, or to buy softer toilet paper.

It sounds like hemmoroids. There's internal and external hemmoroids. One is painful and one is not because there are no pain nerves. He probably has a history of spending a long time on the toilet, not eating a healthy diet, alcohol use, or straining while taking a dump. He should increase the amount of fiber in his diet, not strain while on the toilet and see a doctor if this persists.

Either he is having bleeding piles or is having an ulcer in his rectum if he is seeing blood on the toilet paper. So, unless he can find alternative treatment options, he should consult a doctor. I am answering without reading the other answers and so you might have got similar or better advice.

Lots of possible causes: hemorrhoids, a fissure, polyps, diverticulitis, colitis, Crohn's. Does he often feel tired?
Your friend really must see a doctor, especially since this has been going on so long. Some of those possibilities are serious and some aren't. Your friend may need treatment; he could be in real trouble.

I would guess at haemorrhoids. They don't necessarily hurt, but poohing (especially if you are a bit constipated, which is often the cause of the haemorrhoids to begin with) will make them bleed a bit.
If he's worried - get him to go to the doctor anyway - there are a few nasty bowel disorders which have red blood in stool as a symptom. In combination with long-standing diahorrea, get him to a doctor soon.
