Drug stores should never be selling cigarettes. In some places, in fact, it's illegal for them to do so.

Drug stores also have security arrangements that most people never notice. By placing the dispensary at the back of the store, the staff has the time to apprehend anyone who is acting suspiciously, thus deterring people who are intent on acquiring their drugs illegally.

Placing the dispensary at the back of the store also offers a degree of confidentiality for the customers.

Your question makes complete sense and in a perfect world that would not be the way it was, but unfortunately there are security issues that have to be considered. It is the rotten people ruining it for the rest of us

Pharmacies are generally at the back for two reasons:
1) to afford patients more privacy if they need to discuss anything with the pharmacists.
2) additional security.
