I do not understand why the infection takes 3-4 weeks to abate. Changing antibiotics wont help. The underlying cause has to be found out. Now do you have a lot of pus cells in the urine?
Ask your doctor to perform urine culture test to ascertain what antibiotic suits you better.
It is advisable to see your urologist or nephrologist.
In the mean time will you please let me know the symptoms you have? Do you have pain, burning, irritation while passing urine?
Is there a fever associated with it?
Drink barley water. If you get tender coconut water, have it. Barley water and coconut water are mild diuretics and flush out the toxins from the kidneys. You will pass more urine than normal but it is ok. Dont worry. Diuretics increase the frequency of urination.
Some home remedies are :
Drink lots of water. Cranberry juice has been touted as useful for urinary tract problems, but in some cases it can be an irritant instead of a help. Water is best. Three to four quarts a day is how much you should aim for.
Take vitamin C daily. The dose is variable. Some people can only tolerate 500mg a day, others take 4 to 5 grams without problem; check with your doctor first but a good dose is around a gram a day. This helps keep urine acid and prevent growth of organisms.
Avoid taking bubblebaths. The sudsy water helps break down the protective mucus at the opening of your urethra and allows germs to get in.
For females: after intercourse, get up and urinate as soon as you reasonably can. Urinating rinses away any germs which may have gotten to the urethra.
Avoid thongs. They provide a moist trail right from your anus to the urethra and those E. coli travel right to it.
Do wear cotton underpants or at least cotton crotch panties. Cotton allows moisture to evaporate and prevents having a perfect environment for growth of organisms.
After using the toilet, be sure to wipe from front to back so as not to drag germs from the anal area to your urethra.
Asparagus
Get about 6 asparagus stalks and boil them until they get squashy. Let stand until cool; then eat and drink the juice.
Apple cider vinegar, Honey
Mix 2 tsp of apple cider vinegar with 1 tsp of Honey in a glass of warm water. Drink 6 times a daily!
Cranberry
Drink plenty of cranberry juice at the very first sign of an infection, at least two glass a day. Cranberry juice prevents bacteria from sticking to bladder walls. Try to drink the real cranberry juice, If you cant get the real deal, you can make it by Taking a bag of fresh or frozen cranberries and boil them in water (they will fall apart). Cool and drink. Don't Add Sugar!!! This remedy is also useful for people with kidney problems as well.
Sulfur
Eat foods that are high in sulfur: Spinach, Asparagus, Brussels sprouts, Onions, Cabbage, Beans/Peas, Garlic, Wheat germ, Almonds, etc.
Parsley
Take an entire bunch of fresh parsley and boil in a litre of water. Let cool and drink throughout the day.
Urinary Tract Infection Diet
Increase the intake of vitamins C

It is very important to have a healthy diet which is rich in minerals and other supplements for your daily quota. Avoid irritant foods
A diet which consists of processed food like cheese, chocolate, dairy products should be avoided. You should also avoid spicy food, caffeine, alcohol and cigarettes which otherwise is also harmful. Avoid carbonated drinks like beer, soda or any other drink with fizz.
Stay away from artificial sweetener
Aspartame is one of a kind of artificial sweetener. You will find many artificial sweeteners in the market which should be avoided. It is mainly consumed by diabetic patients and is used in many beverages and other foods.
Other suggestions for Urinary Tract Infection
Hot compression
You can place a heating pad on the lower abdominal area to provide some relief to your pain.
Reflexology
Working on the bladder or kidney points can provide you some relief from this infection.
Dressing comfortably
Wearing loose-fitting and cotton clothes helps a great deal. Wearing tight undergarments and tight fitting clothes creates more room for infection.

Ask your Dr. to send you to a Kidney Specialist.
Not only is a kidney infection miserable, it is nothing to mess
around with. They should check you for protein, and a great
many things to eliminate serious problems. It does seem these days that infections are hard to cure with antibiotics.

I just had a kidney infection. They had me on leviquin for 10 days total. The urologist had put me on this antibiotic. He said that it's the antibiotic he nearly always uses for kidney infections. My dosage was 500 mg, one pill daily. This worked for me by the 9th day my infection was gone, but I continued taking it through the 10th day to make sure.
After all that I just went through (7 days in the hospital) due to my kidney infection and how serious it got, my suggestion would to be to go to the urologist.
I'm sure that your doctor already mentioned that drinking plenty of non-caffeinated and non-carbonated beverages to keep you hydrated is very important. Even though I was drinking 100 oz of water per day, they still had to put me into the hospital because I was dehydrated. So, watch out for signs of dehydration (I had a strong salty taste in my mouth due to dehydration).

Are you drinking water? Or cranberry juice? Try pine needle tea..pick the baby needles from bottom of tree and boil into a tea and drink couple times of day. It is anatural cure for bladder and kidney infections and you can sweeten with honey to make it taste better. If that doesn't help then yes, see another Dr.

What is the current antibiotic you are on? The reason that you may still be having the infection is because of the type of bacteria that is causing it. As far a making an appt. with a Urologist, thats up to you.I would possibly wait until the end of this course of antibiotics and then if not better then go to the Urologist. Also drinking cranberry juice helps.

I hope your doctor did a urine culture, which would show the specific organism in your urine, and then they would know exactly which antibiotic would treat it. Sometimes thats the only way to know which one works. In the ER, we usually do these cultures, but prescribe the antibiotic 'most' likely to help. If he hasn't done this, I would suggest you get one done, it may not show much, since you've been on so many, but, we would do one anyway. Second opinion time? Would he authorize an ER visit? Maybe you do need more testing, so, if it were me, if I wasn't better in a couple of days, I'd definetly get one. Good luck hope you feel better soon,
