form 4506

The tax person should have given you a copy or their office should maintain one but...your tax person must not be a proffessional? I think I would be afraid..very very afraid if my tax person is no where to be found

Request it from the IRS. FYI, you should always keep copies of everything just for cases like this!

You didn't carefully put your copy in a safe place????? You can go to IRS.GOV and get a form wherewith you can obtain a copy from the IRS. Or; you can get a duplicate of your W-2 from your employer and replicate your original filing.
