How do I find an old Provider number for a home health agency that closed 9-10 yrs ago?

A friend of mine worked for a home health agency about 10 yrs ago and it has since closed. She needs the provider number for the company for her records. She does not have any old Oasis records to view. Any help would be appreciated. Thanks :)
