Just the English way, although you have to change the order of the words: designer freelance:
"I'm a freelance designer" = "Sono un designer freelance".
"Designer" can have more suitable translations for some specific fields.
"Progettista" may be one, as said above, but for instance that doesn't apply for fashion or commercial design.
"Indipendente" is formally correct, but in fact rarely used, freelance is common.

"Mi dispiace. Sono troppo occupato per accettare lavori freelance" "freelance" must not be in the Italian dictionary, so maybe seek an alternative word like entrepreneur [if that's the direction you're heading it]

In Italy we commonly use the English wording that's well known as it is among advertising and fashion sectors.
Should you need to translate it into Italian I suggest
"disegnatore indipendente" (for advertising) or
"stilista indipendente" for fashion
