I am in between a C/D in one of my classes at school. And I told my professor that I missed classes because I was in the hospital. I really wasn't but hey, I panicked. So, I need a Doctor's note or some type of verification that I was sick from the 13th-19th of April. Does ANYONE have a layout I can use? Or a real doctor's secretary can fax me one... Thanks!

Nice, you're attempting to cover your lie by doing something unethical and illegal. I'd laugh if this were for an Ethics & Morality class. Haven't you heard anything about honesty being the best policy?

Forging a doctor's note is illegal (at least it is here in the UK). Be honest, tell your professor that you panicked and ask if there is anything you can do to make up for it or any extra work you could do to improve your grade.

Weigh the consequences - a C/D isn't great but it's still a pass. If you get caught lying it will be worse than if you come clean.

No doctor is going to put his/her reputation on the line by covering for your dishonesty, and I doubt anyone that worked for a doctor would risk his/her job for you.

Any idiot knows that if you were in the hospital you'd have tons of papers to verify that (discharge instructions, bills, copies of admission papers that you sign, etc.), and your professor is going to see right through your lies.

Don't compound the problem by adding more lies to the one you already told, or by forging a doctor's name to something. THAT could get you arrested for fraud. (I'd press charges if someone used my name.)

Come clean with your professor. It's the only chance you have.

Making a false medical report like this is he surest way to meet the district attorney.
it isdownright fraud, and no doctor would tolerate it.

I'm curious./ What would you put down for a reason to be in the hospital???

You just flunked"integrity 101." Congratluations.
