The correct thing is ruptured appendicitis because it means that the appendix is inflamed and then ruptures whereas saying ruptured appendix is not correct because an healthy appendix does not rupture.

Any medical condition with "itis" at the end means an inflammation of that body part, e.g. appendicitis. A ruptured appendix means the appendix burst and needs to be removed ASAP.
