They are constantly providing relief after disasters and humanitarian aid. They have several full time plants that are used to produce kits to be used for humanitiarian service. I have actaully volunteered at some of them, and I am always blown away at just how much they are producing to give away. Local wards and stakes also do a lot of service and aid that is not necessarily directed from the church headquarters.

Our church works closely with other rescue organizations, like the Red Cross/Red Crescent, to help people in all disasters.
When people donate items, such as clothes, to the Deseret Industries, some are sold in our second-hand stores, but the best clothes and other items that would be useful are held back in warehouses, and taken out in times of disaster, to be given free of charge to those in need, no matter what religious persuasion.

Relief is provided following every disaster. Following earthquakes in Pakistan the aid was given to an Islamic organization to distribute.
In addition to immediate aid after the tsunami disaster, the church devoted a portion of aid to helping rebuild infrastructure a year later when other aid drys up.

I'm sure that the Mormon Church organizes some sort of relief effort for every disaster. I think all churches do

I don't know about the whole history of the church but the stats page on the official church website shows there are currently 163 humanitarian projects in just as many countries going on at this moment:

The LDS Church (Mormons) are well know internationally for the welfare programs. Per capita membership, you could say that the Mormon Church does more than any other denomination.
