I have been to doctor ive had chest x ray gallbladder ultrsound
bloodwork to check for blood clot and the only thing the ultrasound showed was slight thickness in gallbladder no stones ( I drank water before test) everything has come back normal I am in pain constantly and advil won't help could I have a pinched nerve in my right shoulder My doc says surgery for ballbladder to remove it Im not sure if that is the problem how do I find out without forking out more money for tests?

I believe you may have cholecystis which is an inflammation of the gallbladder. It causes the wall(s) of the gallbladder to thicken. Drinking water before an ultrasound does not alter the test in any way.

Gallbladder attacks usually cause pain that radiates to the back of the right shoulder. I have no abdominal pain with my cholecystis, only chest and upper right back pain.

When you eat fats, the intestines release a hormone known as cholecytokinin that causes the gallbladder to contract and empty its bile content. I would recommend a HIDA scan of your gallbladder to see how well it is functioning. They will inject you with radio-active pigment foll wed with cholecytokinin while taking internal pictures. The ultrasound shows the inside of the gallbladder while the HIDA scan shows its function.

My HIDA scan showed my gallbladder contractions at only 8% vs. the healthy 40%. I have no gallstones and have surgery planned for next week.

I recommend you see a chiropractor. A doctor of chiropractic medicine will be able to tell you if it's a nerve problem or if they think you have a gallbladder problem that requires medical attention . from your description I think it's a Neural muscular problem. This will be the most economical way to find out what you have going on in your body.

Did they do blood work? When I had pain on the right side of my chest, many years ago, they tested my blood to see if my liver was inflamed. This is something to check into, but if it is the liver you will not be allowed to take any type of medication, as it all goes to the liver.

It is possible that you have a stone hidden in a pocket somewhere. This happened to my daughter and it was extremely hard to detect. It took a number of attacks before we had an idea what was going on.

I do not see what pain under your right shoulder would have to do with your gallbladder. Gallbladder causes gastro. symptoms not shoulder pain. You need to see another doctor and do not over due the Advil it will eventually cause gastro problems.
