Things that are infectious 'spread'...from person to person as the carrier comes in contact.
This is a delightful bit of word play pairing the rarely sought after quality of diseases to spread...with the love thought that a smile can do the same.

It's a 'genuine' smile. That is one in which the whole face joins in. That sounds daft but I expect you understand.
It's where the smiler exudes a real sense of pleasure and there is not a hint of 'forced' about it. The body language pays a huge part in this and the viewer, or recipient, is probably quite unaware, conciously, of that element but it certainly contributes.

It means you have a smile that makes others want to smile. Unfortunately you're always going to come across that one sour-puss who isn't happy unless he/she is miserable, so not everyone who crosses your path will smile when they see you smile, but that's not your problem -- it's theirs.
