I ask this for a friend who will have this done and has a "potbelly."

I think they can preform 'key hole' surgery for the appendix now so a small scar no matter how BIG you are. However it is up to the Surgeon, they may well want to have a look around.

Appendix scar isn't actually based from the amount of fat a person has. Your surgeon will be the one to decide on how will he incise for the surgery.

There is what we call a longitudinal mid-abdominal incision which starts just an inch or two before the belly button up to another inch or two lower part of the belly button.

Another is the McBurney's which is an incision done just at the right lower part of your abdomen or just at the same level of your right hip bone. This is at the site just above your appendix.

Having a large amount of fat relates to the depth of the incision not the size.

Of course, during surgery the surgeon will have to go through the skin, subcutaneous, fascia, muscle and finally your abdominal cavity. Your subcutaneous layer pertains to your adipose tissue or as we all know FAT. The thicker the layer of fat, the deeper the penetration should be.

You could talk about the insicion size with your surgeon before the procedure.
