I'm afraid 'the slightest' idea is going to be the best you're going to be able to get on here, and that isn't going to help the cat. Vomiting has many causes, from poisoning to cancer. The fact that the cat is fat may or may not be related - but it certainly won't help the cat's health. Overweight animals are more likely to devlop cancer.
Voice your disgust to your friend. It really is unacceptable not take a sick animal to the vet, especially if he thinks it is dying! Do you know why he won't take him? If it's a cost issue that is no excuse at all. If he's afraid the vet will put his cat to sleep, let him know that vets are not allowed to do this without the owner's permission, so it will be his choice. He could at least gets some meds to make the poor thing more comfortable.
If I was in your position, I'd kidnap the cat and take it to the vet. I'd never have an animal put to sleep without the owner's consent, but I also wouldn't let a person's ignorance get in the way of helping a sick animal.

Ok, my obvious reaction is exactly what you asked us not to do, so....
I will do my best here, but I'm not a vet. Please keep that in mind.
Obviously this is a very ill animal. How old is it? How exactly overweight is it? There are many things that could be wrong here....skin condition - could be mange, could be hormones, could be cancer. Looking "uglier"? Hmm...obviously yet another sign of a serious illness. It may have a impacted bowel, it could have feline aids, leukemia...the list goes on.
Look, if your friend won't take it to a vet, then he is guilty of cruelty to an animal. If you truely care about this cat, offer to pay the vet bill & get it in there. Or, turn your "friend" in the the authorities....OR one other thing. Call your local SPCA, tell them what's up, and have your friend willingly surrender the animal to them. It will get the proper care, or would be humanely put down if there is no hope.
Either way, if neither of you do anything, you are BOTH guilty of animal cruelty. I'm sorry to sound harsh, but drat it all...if you can't afford the vet, you can't afford the pet!
DO SOMETHING...it's obviously in YOUR hands now.

The cat could have anything from mange, to hot spots to allergies to any number of bacterial infections etc ... No one here can provide a diagnosis for you. All people can do is assume.
It's unfortunate that your friend will not take the cat to the vet because by neglecting to provide veterinary care he is violating law and comitting an act of animal cruelty.
You can also report him for neglect anonymously, if you feel that he will not do anything about it.
If you can take him to the vet yourself perhaps that's another idea.
They can provide a skin scraping analysis that will help to diagnose the skin condition. The vomiting is also a bad symptom that should be looked at.

Why does your friend have a pet? The obvious answer is to take this cat to the vet and get some expert advice, if the cat is losing its hair and throwing up something is seriously wrong, whether it is life threatening or not I don't know. If you seriously are worried about this cat and your friend won't take him to the vet why don't you or even better report your friend for animal cruelty because this is cruel and shows that your friend really does not care or love his cat.

Please listen to other people answers too because i am far from an expert!!!!!! The first thing that comes to my mind is flea allergy, if itchy look for dark specks sort of like dirt on the kitty!!!!! Next i think of, is food allergies But i beg you help,your friend know this could be serious.If money is a problem call around to different veterinarian's and see if they have a way you can make payments. If the vomiting is allot cats can become dehydrated. This could be couple of problems or just one!!!!!!!! the skin could also be mange or mites. Skin problems are treatable with the right help!!!!!!!!

Some people just aren't cut out to be pet owners. Your friend is a good example. If proper veterinary care can't be provided, someone should call the humane society. The only thing I can tell is wrong with this cat is its owner.

I'm sorry but the vet is the only one who can tell what's wrong with his cat.If he love his cat and doesn't want him dead,he better take him to the vet ASAP!!!
ps...you said the cat eats a lot,so maybe he's food poisoned...the only option is the vet and like I said ASAP!!!
I hope the poor cat get better soon........

The cat may have allergies causing him to scratch resulting in loss of hair. I've seen it with dogs.
If you see him scratching, that could be the problem. If not, maybe it's his diet and you could try switching food.
