Usually they treat you. But, you can invite and ask to treat them. I sort of think they'll insist on paying for you.

I feel that nowadays, the birthday person tends to organize her/his own party in the absence of family and friends planning one. I also feel that the proper thing to do is for the guests to pitch in for the meal and a little surprise for the birthday person as this wouldn’t be expensive at all. The little surprise should be a small cake the restaurant usually provides or a shot to make the occasion happier.

If invitations were sent the person who sent them should pay for what is described on the invitiation, because by sending the invitation you set an expectation that they should come. If no invitations were sent, then everyone should pay for themselves because you have set an expectation that attendence is totally voluntary and you can't be expected to pay for everyone who shows up. If the latter is true and your friends offer to pay for your meal, then you should accept it as a gift. I hope you didn't send invitations, if you did you're pretty much on the hook for everything you put on the invitation. If you did send an invitation by mistake and because you are poor everyone pays for themselves, you should show them you appreciate them coming by buying them a round of drinks at the end of the night. Party favors are not required for an adult's birthday party.

I think you should treat your friends. It's your celebration therefore you host the party or the treat. Friends are invited to celebrate with you not them celebrate for you. I mean, with all honesty, you're not overly delighted when it's your friends bday. You could be happy but not as much as the celebrant is.
Surprise party is a different story.

Depends on the country unfortunately. I am Irish and it is usually the case that your friends treat you, however I have lived in Turkey and Greece where it is custom for you to treat your friends!!! Good Luck!

For guys is the other way round, invite them for dinner, in return they buy you presents.
Its more advantage if you are a woman on you birthday... male friends who likes you surprise you on the actual day.
