Just give her a card and the family spend the day together.

Take her to dinner. I couldn't afford designer things either. Not for myself let alone an in-law. If she is a reasonable person she will understand that you can't afford those kinds of things. Just do something nice for her birthday to show her she is appereciated and loved. That is the best anyone can do.

Get her a flower have it delivered to her home, also a nice card . You could also get her a nice candle set ,or a gift certificate to a resturant or a movie. A nice box of candy, and a rose. you and your sister inlaw could go in on something together especially a gift card like to her favorite store.

Get her a gift certificate. She's gonna have to accept it and no choice but to spend it at that store. Or a Visa giftcard so she spend it at Prada. Good enough.

i agree, you can try taking her for a nice lunch... or
if she gardens, perhaps you can get her a gift certificate for a garden center? does she like the gym or a sauna or massage?
maybe you can get her an appointment for her nails, or a pedicure?
just some suggestions... i HOPE she'd appreciate any gesture you made to remember her birthday.
take care.

What about a spa treatment? Or if that is too much, just a nice family dinner and a card should do.

Have a family portrait taken of you and your husband (and kids if you have them) Put it in a nice frame. If she displays it or not it is up to her but at least you will have given her something and from the heart.
