If the tile is coming loose because it was defective or poorly installed, your regular home owners insurance almost certainly will NOT cover it.
If this is a new home and you have a 'warranty' on the house, this warranty is actually a type of insurance and it MAY cover the problem (you will have to look to see exactly what is covered and what is excluded under your particular policy).
If the tile is coming loose due to a specific event, your home owners insurance would typically cover the damage _IF_ it covers the event that caused the damage. For example, tile coming loose because it was flooded when a pipe or appliance broke would be covered in many premium insurance policies.
But tile coming loose because it was flooded in a flood would not be covered unless you specifically have flood insurance.
Even if your insurance would cover the problem, you should also look at the cost compared to your deductable before deciding to file a claim. Your insurance company will count the claim against you no matter how large or small it is. So if it costs $600 to fix and you have a $500 deductable, it is almost certainly a bad idea to file the claim (unless you already plan on changing insurance companies in the VERY near future).

NO unfortunately... usually the lifting is caused by a slab crack that has plained (one side higher than the other) caused usually by trees or movement of the ground underneath. you can get some thinset to reinstall your tiles it is very easy. if you want to just jimmy rigg it use liquid nails... clean the area with a vacuum and the back of the tile as good as you can and make sure to use the thin set or liquid nails all over the back and middle stay about 2 inches from the side of the tile so the stuff and move with out oozing out of the sides. grout it and your done. this is really easy.. i have installed a shower and an entire house of tile w/tile base boards. if i can do it so can you! Unfortunately No. Home insurance is designed to cover your building and its contents in the event of damage such as fire, smoke, theft, etc. and damage you might cause to other people. Not for these. In case title lifting was due to flood, then you might get covered.

Highly unlikely. Insurance coverage is for accidents that damage the home, not for minor imperfections (or even major for that matter). If the tiles were lifting due to a flood, they "might" cover it.
