As was already answered, he had polio. But he wasn't necessarily confined to a wheelchair. He would wear braces on his legs for early public appearances and staffers would hold him up.

Franklin D. Roosevelt was swimming in the pond near his home at Hyde Park, N.Y. in 1921. After a period of several days he noticed stiffness in his legs that eventually parallyzed him with poliomyelitis or polio for short.

He had polio starting from August 1921 but now people think that he actually had Guillain-Barré syndrome.
