Whether a Jewish woman marries another Jew or not, any children she has are born Jewish. However, how the children are actually raised depends on what both parents decide, just as with any couple. If the children aren't raised with Judaism and practice another faith, then they are apostates to Judaism, but no matter what, they are still Jews. The difference between them and, say, their non-Jewish cousins is, if they choose Judaism as adults, they don't have to go through the conversion process.

Jewish law recognizes a person as Jewish if their mother is Jewish or if they convert.
As for how they are raised that depends on the parents.

Not necessarily. My sister is Jewish and her husband is Catholic. Their daughter was brought up Catholic.
Try to tell her she is Jewish and she will laugh it off. The Jewish religion may recognize her as Jewish but what if she doesn't practice it?

No. The two clever, tolerant parents discuss the matter and decide. There is no RULE which says that the mother's religion should decide.See for reference:

Goes by the mother if you're talking about Judaism.
For Christianity it goes by the father.
