How about an email to set up a good time, and give an indication of what you'd like to address, and then a phone conversation. Something like:
email: Hi I've been thinking a lot about ...(our falling out, or what I did the last time we were together, or the way that I hurt you).. and would like to express...(my regret for having hurt you, etc.). May I call you to talk about this? If there is anything you need to say to me or ask me, I promise to listen to you. If so, please let me know when to call.
phone call: speak from your heart, and let the other person express themselves fully too.
