There is no particular 'meaning'...

It is an open and very polite question asked by someone who perhaps has lost contact with you / or just hasnt seen you an ages...

What do you do for a job now? / Have your situations changed since we last met? / are other ways of saying it I guess....

It politely means "how great or terribly crappy is your life compared to mine right now". People just don't say that.
