You really shouldn't be playing any very physical sports unless you have a somewhat recent tetanus shot. Tetanus is a lot scarier than needles. You can't go forever without seeing a needle. I think this is a great opportunity to kill 2 birds with one stone. You can play varsity tennis AND overcome your fear of needles. Good luck!

I don't think so because the high school sports association of district's regulations and rules (NOSSA in OFSSA in Canada) doesn't have such an article. I think they only do that in the professional circuits to make sure a person isn't drugged, drunk or doing dope. So don't worry, you don't have to get physical shots. Even my school's NOSSA representative said so too, and he knows the book by hand. They only check to see if you have an illness or something similar to that (like asthma), so that they can be prepared in case the need for emergency aid should arise.

haha, so do I. I don't think you HAVE to. I never knew that was a requirement for the highschool tennis teams... Ask your coach about it.
If worse comes to worse and you have to get a shot, you can just ask your parents to write a note saying its... "against your religion" or some other odd excuse...

No, but you have to have the physical. If your immunizations are already up to date, there's no reason for you to get any extra shots.
